New student registration requires the following documents:

Note: We can make copies of your originals if needed.

1. Birth Certificate
2. Social Security Card
3. Immunization records (shots)
4. Copy of parent’s High School diploma (or GED) (For study at home students). If a higher-level diploma is available (vocational, college, etc.) we will need a copy of it as well.
5. Funds for Registration and Tuition as agreed to

4. Guidance Counselor’s name

C.O.A. will provide for you on your enrollment date:

1. Notification to your previous school of your enrollment here.
2. Request Permanent records from your previous school.
3. Provide you the form to notify the Public School Superintendent of your intent to Home School.

There may be a photo taken of the student for the permanent record file.
